You have now passed your exam, paid your fees and are ready for the next steps.
Step 1: W-9
As a newly licensed individual, you are now eligible for receiving commissions. In order for you to be paid, we must have a W-9 form filled out and on file with the home office.
Step 2: E&O Insurance
As a life licensed individual, and as a member of Common Sense Financial you are required to have Error and Omissions insurance with the company. In order to get appointed with companies to represent their products, they also require you to be covered by E&O. You can sign up for a monthly subscription ($25 monthly). Once you have purchased the policy, you will receive an update for the policy when you are added to the group policy. It is not automatic yet.

Step 3: Anti-Money Laundering
In order to be appointed with insurance companies, you must have a current Anti-Money Laundering course completed. If you do not have a current course certificate, you may obtain one from the following provider.

Please register for a new account in SureLC for our life providers. Life providers and F&G Annuities

***If you have used SureLC with another marketing group, you can use your same email address, but a different password will be required.

Complete the Create Your Account steps

Once complete, you’ll see a confirmation message stating “Your account was successfully created”

Next, you’ll receive a confirmation email at the address entered as your Login ID

The email will be from setup@surancebay.com

Locate the email and click the link to activate your account

Complete the Account Registration process

Log in to SureLC

Review all sections listed under My Profile and make updates, if necessary

The profile sections are located on the left-hand side of your screen starting with My Profile and ending with Profile Documents

All sections with a Red dot will require additional information before you are able to create a new contract request

To begin your contract request, click Contracting Requests

Click Create Request- Select carrier

Complete the information and click the Next button to advance through each section of the request process

When you reach the Review & Sign step, read and scroll through each page of the carrier contracting forms

Once you reach the bottom of the last page, click Apply My Signature

Read the signature attestation statement, click Sign to complete and submit the contract request

To submit additional contract requests, return to Contracting Requests and repeat Step 8

- Please register for a separate Annuity account.
- Follow the same basic steps as you did for starting your life insurance appointments, but now for Annuities outside of F&G. You may use the same email, but the password will have to be different.
Step 5: Review Code of Professional Conduct for Agents
We expect you to represent Common Sense Financial with the highest level of professionalism.
Please review the Code of Professional Conduct and bring any questions or concerns to your Partner.
Step 6: Practice filling out life applications
After you have received notice that you have successfully be appointed, you should now familiarize yourself with the online apps for both Allianz Life and F&G. Here are two training videos that walk you through how to fill out the online application.
